What is vitamin B12?
Vitamin B12 is a water-soluble vitamin that contains the mineral cobalt. The body needs it to help produce healthy red blood cells, synthesize DNA, and maintain normal nerve cells. B12 also participates in the metabolism of homocysteine, an amino acid found in the blood.
Absorption is a multistep process. Stomach acid and enzymes first release food-bound B12. The vitamin then attaches to intrinsic factor, a protein made in the stomach, before being absorbed in the final part of the small intestine. Problems at any stage—from low stomach acid to autoimmune loss of intrinsic factor or intestinal disease—can reduce absorption.
Vitamin B12 benefits: established roles and common myths
The strongest evidence supports preventing or correcting deficiency. Restoring B12 in a deficient person can improve abnormal blood counts and may resolve symptoms such as fatigue, weakness, tingling, or balance problems, depending on their cause and how long they have been present. Neurological recovery may be incomplete when deficiency has continued for a long time.
Vitamin B12 is frequently promoted as an “energy vitamin.” It is involved in normal energy metabolism, but it is not a stimulant and does not directly provide calories. A supplement may help fatigue caused by B12 deficiency; it has not been shown to reliably increase energy in people who already have adequate status.
B12 supplements can lower elevated homocysteine, especially in deficient individuals, but reducing a laboratory marker is not the same as proving prevention of heart attacks, dementia, or other diseases. Evidence for routine supplementation to improve cognition or mood in B12-replete people remains inconsistent. Claims that B12 causes weight loss, detoxifies the body, or prevents many unrelated conditions are not supported by robust evidence.

Supplement labels often list amounts far above the RDA because only a small percentage of a large oral dose is absorbed without intrinsic factor. Unfortified plant foods do not provide reliable, biologically active B12. Spirulina, seaweed, fermented foods, and nutritional yeast should not be assumed to contain useful B12 unless the label specifically confirms fortification. People following vegan diets generally need a dependable fortified food or supplement strategy.

Vitamin B12 deficiency symptoms
Deficiency can develop gradually because the liver stores substantial B12. Early symptoms may be subtle and nonspecific. Possible signs include fatigue, weakness, pale skin, shortness of breath, a sore or smooth tongue, reduced appetite, or unintended weight loss.
Neurological and cognitive features can occur with or without anemia. These may include numbness or tingling in the hands and feet, reduced vibration sensation, balance problems, difficulty walking, memory changes, confusion, mood changes, or visual symptoms. Because many conditions can cause these complaints, symptoms alone cannot diagnose B12 deficiency.
Who is at higher risk?
- adults over 50, because lower stomach acid can reduce release of food-bound B12;
- people following vegan or highly restrictive vegetarian diets;
- people with pernicious anemia, an autoimmune condition that reduces intrinsic factor;
- people with celiac disease, Crohn’s disease, or other conditions affecting the stomach or small intestine;
- people who have had gastric bypass, bariatric, or other gastrointestinal surgery;
- people taking metformin or long-term acid-suppressing medicines;
- pregnant or breastfeeding people with low intake, and infants born to or breastfed by a deficient parent.
Infants and young children can experience serious developmental consequences when deficiency is missed. Dietary patterns, maternal status, growth, and symptoms should be reviewed by a pediatric healthcare professional rather than managed with an adult supplement.
Testing and diagnosis
A clinician may start with a complete blood count and serum B12 level. If results are borderline or do not match the clinical picture, methylmalonic acid (MMA), homocysteine, folate, or other tests may help. MMA typically rises when cellular B12 is inadequate, but kidney function and other factors can affect interpretation.
There is no single perfect test or universally accepted cutoff. A person can have neurological symptoms before marked anemia appears. Diagnosis therefore combines symptoms, examination, diet, medicines, medical history, and laboratory results. Testing for antibodies related to pernicious anemia or investigating gastrointestinal disease may be appropriate when poor absorption is suspected.
Supplements, injections and safety
Common supplemental forms include cyanocobalamin and methylcobalamin. Cyanocobalamin is stable, well studied, and widely used. Methylcobalamin is biologically active but has not been proven universally superior. Sublingual products are not consistently better than standard oral tablets.
High-dose oral B12 can work for many people because a small amount is absorbed passively. Intramuscular injections may be chosen for severe deficiency, prominent neurological symptoms, significant malabsorption, adherence concerns, or rapid replacement. A 2024 network meta-analysis found that oral, sublingual, and intramuscular routes all raised B12, with no statistically significant differences in the primary comparison; individual treatment still depends on the clinical situation.
No tolerable upper intake level has been established for vitamin B12 because toxicity appears low in healthy people. That does not make unnecessary megadoses risk-free or useful. Supplements may cause acne-like eruptions or other reactions in some people and can complicate laboratory interpretation. B12 can also interact with certain medicines or be unsuitable in specific rare conditions.

- Know the goal: routine dietary coverage and treatment of confirmed deficiency are different uses.
- Check the amount per serving: compare micrograms, serving size, and frequency rather than package claims.
- Choose a suitable format: tablets, liquids, chewables, and injections differ in convenience and clinical purpose.
- Look for independent quality verification: recognized third-party testing can help confirm identity and manufacturing quality, although it cannot prove health benefits.
- Avoid unsupported promises: be cautious of products marketed for instant energy, weight loss, detoxification, or disease prevention.
